Introduction and Basic Services

Castlewood at Monarch Cove is located in picturesque Pacific Cove, California. It is about 75 miles south of San Jose and 10 minutes away from the Monterey Regional Airport. Monarch Cove treats both men and women with eating and co-occurring disorders via a residential program, a Partial Hospitalization Program (PHP) and an Intensive Outpatient Program (IOP).

Facility and Meals

The homey residential facility, which houses a maximum of 12 clients, is located near the coast and has a beautiful garden out front. It is bright and colorful, with a dining area stocked with fresh fruit and flowers. Double rooms have large beds; private rooms are available based on medical necessity. There is an indoor lounge area with wooden bookshelves, ample seating, a TV and a stone fireplace, as well as lounge areas outdoors. Treatment offices have comfortable leather seating and a view of the surrounding trees. Trained chefs prepare nutritious meals for clients. Smoking is allowed in designated outdoor areas.

The outpatient offices, called Ryan Ranch, are located in nearby Monterey. The facility is a Spanish-style villa with a well-appointed dining area, a stocked kitchen, a latticed outdoor lounge area and comfortable, warm clinical rooms.

Optional, ocean-adjacent transitional living for PHP clients is located 11 miles from where treatment is provided. Three female-only apartments have two double bedrooms each and one male-only apartment has the same capacity. Transitional living is offered at an additional cost. Transportation is provided if needed.

Treatment Protocols and Team

Because of the complexities of eating disorders, treatment at Monarch Cove is highly individualized and differs for men and women. The length of stay depends on the individual, but is 36 days on average.

The program begins with highly monitored weight stabilization, during which the client is eased into the regular treatment program. Modalities include CBT, DBT, schema therapy, creative therapies, EMDR, mindfulness training, identity development, healthy relationship education psychotherapy, relapse prevention therapy and a focus on nutritional education. Clients also participate in co-ed group sessions. Each week residents have one session with the psychiatrist; four individual sessions with their primary therapist for their first month; two individual meetings with a dietitian and additional sessions as needed for co-occurring disorders. In addition to group nutrition education there are therapeutic discussions after every meal. Off-site 12-step meetings are available.

The daily residential schedule revolves around meals and starts at 6 am with a weigh in and the taking of vitals. Breakfast is served from 7:30 to 8:30, followed by individual sessions, group sessions or free time until approximately 10:30 am. A snack is served until 11 am. There is then another group session until lunch at 12:15 pm, and a mix of group and individual sessions until a 3 pm snack. Individual sessions run until 6 pm. Residents then have dinner and free time, an outing or a 12-step group. Another snack is given at 8 pm, and lights out is at 10 pm.

Clients in the PHP may participate up to seven days a week, six hours a day. Treatment includes three weekly sessions with a primary therapist, one with the staff psychiatrist and one to two with a licensed dietitian. Life skill coaching is also provided, as are six hours of daily group therapy, one or two staff-supported meals with post-meal processing and relapse prevention planning.

IOP clients can receive treatment up to five days a week with flexible program hours. They participate in 20 hours of group therapy weekly; small therapeutic support groups; weekly individual sessions with a dietitian; two weekly individual therapy sessions; up to five supervised meals with post-meal support and continued development of relapse prevention skills.

The clinical staff includes a psychiatrist, an LPCC, an LVN, an LMFT, an MFT, an MFT-I and dietitians.

Aftercare involves a relapse prevention plan, continued care and participation, if desired, in an alumni program.

Monarch Cove offers family therapy including Family Weeks every other month, which help establish a strong support system for clients. Family Week is held over the course of three days and teaches establishing healthy boundaries and communication skills. Families also participate in experiential sessions, individual therapy and multi-family educational sessions.
