HMO Plan Basics

An HMO, or health maintenance organization, is a type of managed care plan. These plans have several distinguishing features, which are detailed below:

  • You must choose a primary care physician. HMO plans require you to choose a PCP. You cannot receive any treatments or visit a specialist without a referral from your PCP.
  • You pay a co-payment for all services. When you have an HMO, you will typically have a copay for all of your covered services. This means that you will be required to pay a flat fee at the time of service.
  • You must use in-network providers. HMO insurance plans will cover services only if they are provided by doctors and facilities that are part of the HMO's network. If you use providers outside of the network, you will be responsible for all of the associated expenses.

What Does This Mean for Addiction Treatment?

For example, in order for your treatment or any visits to a specialist to be covered, you must first obtain a referral from your PCP.
